Registration for the 2024/2025 preschool year starts on January 10th for Tree House alumni and January 17th for new families to Tree House.

Monthly fees paid by etransfer on the 1st of each month.

*$150 non-refundable registration fee, $150 refundable program fee if withdrawn by Aug 1st, 2024.

As a parent/guardian of the above registered child, who will attend the Tree House Playschool, give permission to the staff of the Tree House Playschool to: *

(1) Take my child on scheduled fieldtrips with notice, and/or supervised walks, visits to the playground and/or gymnasium, with or without notice, on or off the school premises during scheduled class times;
(2) Seek or provide appropriate medical care and/or emergency medical treatment (911) in case of emergency, advising parent/guardian or emergency contact person if parent(s)/guardian is unable to be reached, as soon as is practicably possible;
(3) Release my child's registration and/or emergency information (contained on this form) to the Greater St. Albert Catholic School Division, Alberta Health Services and/or emergency responders ONLY for the purposes of emergency procedure, protocol, health or safety;
(4) Allow my child to be photographed or digitally recorded in a classroom or fieldtrip setting (non-identifying imagery may be used on Tree House website or social media).

Please be aware that Tree House Playschool assumes no responsibility or liability for images or third party use of images posted on social media or elsewhere.

I am aware that a $150 registration fee (non-refundable) and $150 program fee (total $300) are due to complete registration. I acknowledge that monthly fees are due by the first of each month of the program year (except by Sept 15th). I accept that 30 days written notice is required for withdrawal from the preschool program and where not given, full monthly fees are due. *
